    E1-11 Forms/Cards

    Has everyone going to Malmo/Copenhagen got their E1-11 form/card?? Just remembered mine today......

    For those not knowing, its a medical form enabling you to receive free medical care if needed when abroad in the E.U. or something like that anyway.....

    Can be got from the Southern Health Board on George's Quay (Corus Building) 3rd floor......

    All you need is...

    1.Passport
    2.Plane Tickets/Travel Insurance
    3.Credit Card
    4.CCFC Shirts/Colours

    No.3 can be used for anything you've forgotten (and for tinnies and spirits in Tax Free shopping )

    Anyone travelling anywhere within Europe or Switzerland should get themselves a European Health Insurance Card
    It's free and handy but hopefully none of you will have cause to use it.

    From 1st June, 2004 the E111 and a number of other 'E' forms (i.e., E128, etc.) have been replaced by a new European Health Insurance Card.

    they'll give you a stamp saying that you have ordered one and its on the way.......they'll also give you the applciation form to fill out at a later date...tis well good like..........

    bring in something with your pps number on it......tax credit form, p60 etc...you have until 5pm........

    now do what you're told........!!!
