My list has 1978/79 Final: Cobh Ramblers v Limerick, not played due to a dispute over venue. I haven't gone back to the papers to check it, might get a chance to look at De Paper tomorrow.
BTW, that would have been the third time a Final in the MSC wasn't played due to some dispute or other. Limerick were listed as the Away team in each case (1939/40, 1957/58 & 1978/79).
There was one more: 1998-99: Cork City v Waterford United, awarded to Cork when Waterford refused to play a delayed Final (info from corkcityfc.ie).

Cobh were awarded the 1978/79 trophy after Limerick refused to travel to the final. Makes 2016 the club's 5th title
